Who We Serve

About Scotland Regional Hospice

Welcome to Scotland Regional Hospice

Since 1986, Scotland Regional Hospice has been committed to providing exceptional, compassionate end-of-life care to people in our community. A team of professionals and volunteers, including physicians, nurses, social workers, chaplain, and bereavement counselor, provides personalized care for each patient and family we serve.

Hospice care is for patients who are considered to be in the last 6 months of life, as certified by a physician. Our care focuses on pain control and symptom management. A team of professional staff members provides personalized care for each patient. Patients receive care in their homes, in rest homes, assisted living and long-term care facilities, as well as hospitals.

People We Serve

The patients and families we serve must reside in our
service area, which includes all of Scotland County and
areas of Robeson, Richmond, Hoke, Marlboro, Dillon and Chesterfield Counties.

Hospice patients are cared for in the familiar surroundings of their own homes, or home-like
settings such as rest homes and nursing homes.

The Hospice team develops an individualized plan of care based on the needs of each of our patients and his or her family members.

Those eligible for hospice care include persons of any age with a life threatening illness and limited life expectancy.

The patient’s physician must certify that the individual is appropriate for hospice care.

The patient must understand the hospice philosophy and agree to admission.

Referrals may be made by the physician, patient, family, friend, minister or other community agencies.

Once the physician approves the referral, a Scotland Regional Hospice staff member will schedule a meeting with the patient and family or caregiver to explain the program and answer any questions.
